LAWRENCEVILLE – The Cumberland Cape Atlantic YMCA and the New Jersey Partnership for Healthy Kids-Vineland were among the winners of the New Jersey Department of Health’s first Population Health Heroes Awards, which were presented at the 2017 Population Health Summit at Rider University. Awards were presented by members of the Population Health Action Team, which includes Cabinet officials of the Departments of Agriculture, Children and Families, Community Affairs, Environmental Protection, Health, Human Services and Transportation.
“The goal of these awards is to recognize and showcase the work of an individual, group, business, municipality, county, faith-based or other community organization that have successfully implemented a program or intervention that has made a measurable difference in health outcomes for New Jersey residents,” said Health Commissioner Cathleen Bennett.
